The accident report of an Air India Express jetliner which crashed upon landing in Kozhikode last year shows a complete breakdown of basic rules which exist to prevent accidents. There were huge mistakes made by the pilot, incorrect information provided by Air Traffic Control and extremely poor management by the airline. Twenty-one people were killed in August last year when the Boeing 737-800 slid off the table-top runway and broke into three parts. Luckily the aircraft did not catch fire.
