Sources in the fire department have told NDTV that the last audit at the Maharashtra Mantralaya took place in 2008 whereas they should take place every year. The report pointed out that fire extinguishers and alarms were not working, also there were illegal constructions inside the building. Worse still, the fire brigade's initial inquiry after the blaze reveals. These recommendations were not acted upon.
